Preparing for adulthood (PFA) and transitions

Transition is the change from something familiar to something different. You may hear the transition referred to as "preparing for adulthood" or "PFA".
We go through lots of different transitions in our lives. Some of the big transitions you will go through include:
- starting at a new nursery, school or college
- moving house
- changing from children's support services to adult support services
Your school, health care and social care services will help you prepare for adulthood. This will focus on 4 areas:
Education, training and employment. Your decision to go into further education, get training or get a job.
Independent living. Helping you to make your own decisions and choices. This includes where you live, how you manage your money and changing your environment to meet your needs.
Community inclusion. Being a part of your local community and having friends.
Health and wellbeing. Helping you to be as healthy as possible in your physical, mental, sexual and dental health. This includes access to sports activities and annual health checks.

Special educational needs and disability (SEND) local offers provide a wide range of advice and support for you and your family.
SENDIASS offers
Special educational needs and disability information advice and support service (SENDIASS) helps parents, carers, children and young people, providing support through the SEND processes.
