Health and care passport

A health and care passport is a document that quickly summarises your needs, likes and dislikes.

Health and care passports are also known as:

  • NHS passports
  • patient passports
  • health passports
  • hospital passports
  • 'My health and me' documents
  • 'All about me' documents

Health and care passports can be used by any medical and health care professional. This includes a GP receptionist, a doctor, a nurse, your dentist or an ambulance crew. They can be used for planned appointments and emergencies.

It gives medical and social professionals more information about you. It makes the information quick and easy to read for professionals so they can give you the right care and treatment.

Check and update your health and care passport regularly. Make sure to write any changes in your health or care needs and likes and dislikes. This helps professionals to provide a better and more person-centred care.

There is not a standard health and care passport template. There are lots of different templates available online.
