Referral information

The 24/7 advice telephone number is for anyone requiring palliative care advice or support. This service operates 24 hours a day, 365 days a year. It can be used by patients, healthcare and social care professionals, carers, friends or family members.

You do not need a referral to the Community Specialist Palliative Care team to access the telephone advice service.

Welcome to our service

Palliative care provides people with life-limiting illness with the best quality of life. It also supports their families and carers.

We are a community-based team, working across most of Norfolk. Our team will become involved if your needs are more complex.

We do not provide direct care. Our team will work with you and your primary healthcare team. We will liaise closely with others involved with your care.

We provide:

advice on pain management
advice on managing symptoms like nausea, vomiting and breathlessness
complex emotional, psychological and social support
advance care planning (discussing and considering your future care preferences)
an opportunity to talk about the effect your illness is having on you and those close to you
information about other services that may help your decision making

Our team has healthcare professionals with different areas of expertise. Our team specialises in providing palliative care. We work with patients, families and carers. We also help other professionals to develop their expertise in palliative care.

Our team includes:

specialist palliative care nurses
specialist palliative care doctors
physiotherapists
occupational therapists (OTs)

Information and advice

Intravenous (IV) iron therapy

Intravenous (IV) iron is an infusion of iron directly into your bloodstream. We use it to treat iron deficiency and anaemia.

Referral

We will only accept referrals from health and social care professionals.

For advice about referrals and requests for admission, call 0330 158 8011 and select option 1 (advice for health and social care professionals).

Referral process for patients

A specialist triage nurse will assess your referral from the health or social care professional.

As part of the assessment they will need to complete an initial telephone assessment. This is to ensure that we can identify and understand your individual needs. We want to make sure you get the best possible person-centred care.

The specialist triage nurse will contact you or a nominated carer or family member (with permission). They will ask you several questions.

After the assessment, we may decide that our service is not the most appropriate for your needs. You may need to be referred into a different service to get the most suitable care for you. We will discuss this with your healthcare professional. We offer copies of any letters written following your first assessment.

You will be contacted to arrange the next steps.
