Welcome to our service
Palliative care provides people with life-limiting illness with the best quality of life. It also supports their families and carers.
We are a community-based team, working across most of Norfolk.
Our team will become involved if your needs are more complex. We do not provide direct care. Our team will work with you and your primary healthcare team. We will liaise closely with others involved with your care.
We provide:
- advice on pain management
- advice on managing symptoms like nausea, vomiting and breathlessness
- complex emotional, psychological and social support
- advance care planning (discussing and considering your future care preferences)
- an opportunity to talk about the effect your illness is having on you and those close to you
- information about other services that may help your decision making
Our team has healthcare professionals with different areas of expertise. Our team specialises in providing palliative care. We work with patients, families and carers. We also help other professionals to develop their expertise in palliative care.
