Welcome to our service

Our Luton Community Matrons and District Nursing Service provides nursing treatment and care to the people in Luton.

We provide care for patients with a wide range of medical conditions and aim whenever possible, to promote independence and encourage self care.

Our team work closely with all of the services involved in your care, to help you stay well and living comfortably at home.

Community matrons

Community matrons provide advanced clinical assessment and intensive case management (coordination of care) for patients who have more than one long term condition such as diabetes, coronary heart disease and arthritis.

Community matrons also support the families of people with long term conditions and often work closely with hospital consultants, GPs, other nursing staff and social workers to ensure people get the help they need.

The Community Matron Service incorporates a team of experienced nurses who support patients with long term conditions, closely monitoring their well-being in order to prevent unnecessary hospital admission.

The nurses work autonomously with advanced clinical skills and prescribe for their patients if necessary, making appropriate referrals when required, acting the coordinator of care for all health professionals involved in the patients care.

The service is provided 7 days a week, 24 hours a day working closely with the Out of Hours Community Nursing team to provide cover during the evening and nights.

Patients are encouraged to call their community matron in the first instance as soon if they feel unwell or there is a change in their health status.

District nursing

Our District Nursing Team will assess, plan and provide nursing clinical care to those people who are often housebound due to ill health, either in their own home, or in a care home that does not provide nursing.

We will see patients in their place of residents, such as their home or residential care home.

Examples of district nursing care

We provide a wide range of services. We work collaboratively with GPs, social services, and a range of other hospital and community health services.

Routine care

This includes diabetes care, post operative care, simple or complex wound care, leg ulcer management, catheterization, bowel management and management of patients requiring enteral feeding.

Complex care

This includes end of life care to patients with cancer and palliative care needs.

Scheduled care

This includes planned interventions as identified in the patient’s care package.

Unscheduled care

This includes interventions required in addition to the identified care package for example catheter blockage in the out of hours period, administration of medicines including intravenous or central line therapy, syringe driver and injection.

Referral

We accept referrals from all healthcare professionals. Please note, we require a signed consent form by the patient to agree to accept the service.

Community matron referrals

Please be aware, we require a signed consent form by the patient to agree to accept the service. 

The service is for adults aged 18 and over who registered with a Luton GP. 

They must meet 2 or more of the following criteria:

  • Recent exacerbation or deterioration of chronic illness within last 90 days
  • More than 2 falls within last 2 months
  • Recently bereaved and at risk of medical decline
  • Cognitively impaired and medically unstable
  • Taking 8 or more medications
  • 2 or more hospital admissions within a 12 month period
