Norfolk Urgent Community Response
Our service provides short periods of 24-hour care to patients in their own homes. This is to avoid unnecessary admissions to hospital and, where appropriate, facilitate early discharge from hospital.
Referral
We accept referrals from healthcare and social care professionals.
When making a referral, you will need to have ready the patient's GP surgery details and all relevant patient information.

This service cannot be contacted directly. You can contact them through the single point of contact service. This is for registered patients and health and social care professionals.
The single point of contact service is open Monday to Sunday - 8am to 6pm, including bank holidays.
Please note, it is an administrative service only. They are unable to provide clinical advice or information on nursing schedules.
