Norfolk Rapid Assessment Team

We provide rapid assessment for West Norfolk patients who have been admitted to hospital but want to remain in their own home. We assess and organise appropriate support.

Our team includes a physiotherapist, occupational therapist, two specialist nurses, plus a technician. 

To assist with safe and timely hospital discharge, patients are referred by emergency care and ambulance services for assessment. Patients can request an additional assessment while the patient is in hospital.

Interpretation and translation services can be arranged.
