Norfolk Heart Failure Service
This service provides assessment and support to help heart failure patients manage their long-term condition. The service also provides monitoring during the treatment phase and follow-up thereafter by telephone or home visit.
Referral
We accept referrals from healthcare professionals.
If you have used this service before, you can self-refer after being discharged from the service.
If you feel that your condition is worsening while you are waiting to be contacted, please speak to your GP to expedite your referral.
