We accept referrals from GPs and care providers. You will need to send a completed referral form to your local Learning Disability Team.
Eligibility
We will accept accept referrals for people with a learning disability (and a dietary need) who meet two or more of our criteria:
- Unable to have their mental health or physical needs met through mainstream service provision with support and reasonable adjustments.
- Presenting behaviours that challenges, where the behaviour is of the severity and frequency to cause significant risk to self, others or the environment.
- Requires an integrated specialist learning disability team approach to care and other learning disability health professionals are involved at the same time.
- The learning disability is impacting their ability to engage or comply with therapy or treatment. For example, it has had an impact on most areas of their live, significantly reduced their ability to understand new or complex information, to learn new skills or reduced skills to cope with independence in everyday life.
