This service is offered from Norfolk and Norwich University Hospital.
We help the safe transfer of patients into a community unit or directly into their home with occupational therapy equipment and interim support. We help avoid unnecessary admissions into acute hospitals.
Our team assesses patients needs and decide the appropriate service within our Trust to support a safe transfer. We provide a 365-day service, serving the acute hospitals and GPs in Norfolk and Waveney.
Our service has two aspects:
Admission avoidance - 8am to 8pm
Bed-based community hospital unit admissions - 8am to 6pm
Priority is given to palliative patients and admission avoidance patients. We liaise with our Community Inpatient Unit colleagues and fellow health and social care colleagues across the Norfolk and Suffolk Integrated Care Board (ICB).
We care for patients who are:
attending the Norfolk and Norwich University Hospital A&E department and assessment units
admitted to the Norfolk and Norwich University Hospital wards who require rapid discharge support and/or rehabilitation
