Luton Integrated Community Diabetes Service
We are a multidisciplinary team. Our team has specialist diabetes nurses, dietitians and support workers. We help to optimise diabetes management and reduce risk of diabetes related complications. We run specialist diabetes clinics and have lots of diabetes education programmes.
Our service is open to patients over the age of 18 years old diagnosed with type 2 diabetes.
Our Integrated Community Diabetes Service provides a range of services including:
We support service users in clinic and on the telephone. We hold regular clinics at Luton Treatment Centre.
The specialist nurse reviews are for people who meet our referral criteria. This includes:
- being on maximum tolerated oral medication
- being on an injectable therapy to manage their diabetes
You will have a regular review from a diabetes specialist nurse. The frequency of these reviews will depend on your clinical needs.
Depending on your clinical needs, a diabetes support worker may do the supportive clinical duties.
The specialist nurse can review patients in their home if they are housebound. This is when you are unable to leave your home to go to your GP practice or the diabetes specialist nurse review clinic.
This is a not the same as the housebound routine diabetes review.

This is a regular review with a specialist dietitian. They will give you advice to help you manage your diabetes through diet and lifestyle. The frequency of these reviews will depend on your clinical needs.
The specialist dietitian can review patients in their home if they are housebound. This is when you are unable to leave your home to go to your GP practice or the specialist dietitian review clinic.
This is a not the same as the housebound routine diabetes review.
Routine annual diabetes reviews are usually completed by your GP. Some people are housebound and not able to leave their homes to go to their GP surgery.
We offer a routine annual diabetes review for housebound people with diabetes. A diabetes support worker will complete the annual diabetes review with you in your home.
You will need a referral to access this review. This service for people with diabetes who are housebound.

We accept referrals from:
- GP practice teams
- specialists nurses
- hospital diabetes teams
- self-referrals from people registered with a GP surgery (only for Desmond training and education)
