Welcome to our service
The Integrated Discharge Team have the lead for admission avoidance in the role of GP liaison by discussing with referrers patients who may require a same day assessment by the acute trust.
The team advise and suggest possible alternatives to a hospital attendance and also have a role in identifying and developing patient pathways and community services.
We work with other organisations as part of the integrated discharge team based at Luton and Dunstable Hospital.
The team also have responsibility for facilitating complex discharges from the Luton and Dunstable Hospital.
The services our team provide include:
- assessing and prescribing of equipment
- facilitating and supporting complex discharges with health funding
- referring to community services
- advising and supporting hospital staff, patients and carers about community services
- liaising with voluntary and statutory services
