Referral information

Welcome to our service

Our team is made up experienced nurses. They are able to provide acute (short term) medical and nursing care in patients own homes, residential care homes for the elderly or an intermediate care setting to prevent hospital admissions, or facilitate early discharge.

This team is part of the Luton At Home First Service.

Our services include:

assessing, supporting and nursing care to patients in their own homes including care homes and if required for those needing a period of intermediate care or rehabilitation beds
providing a first point of contact for advice and nursing assessment for acute conditions to people living in residential care homes in Luton
assessing patients, delivering acute intervention such as intravenous (IV) drug therapy, and monitoring patients during acute episodes of illness
working collaboratively with GPs, consultants and other services such as the community assessment and rehabilitation team and social services

Referral

This service is provided to adults aged 18 years old and over who are registered with a Luton GP.

We accept referrals from:

GPs including out of hours GP services
hospital consultants and the hospital clinical navigation team
social workers
staff working in residential care homes in Luton
ambulance personnel
